Can I train Hitpoints directly?

No — Hitpoints XP is earned passively at 1.33× your combat XP rate (for every 3 Strength XP, you earn 4 HP XP). The only way to raise HP level is through combat.

Is 99 Hitpoints worth pursuing?

It's mostly a side effect of maxing other combat skills. By the time you reach 99 Attack, Strength, Defence, Ranged, and Magic, your Hitpoints will often be 95+ from passive accumulation.
